This form is used to certify events hosted by Hunter College faculty or staff, taking place on campus, in order to ensure that all guidance from Federal, State, City and CUNY entities is being followed for the safety of all participants. Please submit at least two weeks prior to your scheduled event.
The responsible party is the person on-site for the event. This person is responsible for ensuring the event complies with Hunter’s General Safety Guidelines. This may be the same as the requester listed above.
