AS A FACULTY MEMBER, I CERTIFY TO THE FOLLOWING (Please ensure all items are certified): This form is being filed PRIOR to the commencement of the classroom activities; The data from these projects WILL NOT be used for presentation at conferences, publications or doctoral dissertations without futher IRB review; No identifying data will be collected (no names, social security numbers or other codes that can be identified or linked to subjects). The projects will not use or disclose health related information; It is my responsibility to ensure that research practica are conducted according to ethical standards: respect of persons, beneficence and justice. (https://www.hhs.gov/ohrp/regulations-and-policy/belmont-report/index.html); No student research practica under my supervision will collect information regarding illegal activities; information that could cause emotional distress to other people; information which would place the student at risk; conduct research which would put the person at risk if confidentiality were breached; I will ensure that all students have completed their CUNY HSR Social and Behavioral CITI training; In the event that any of the student projects do not meet the above criteria, (e.g. collection of data for research purposes), I will forward the materials for HRPP/IRB review and approval before the study can commence.
