1. This application form is intended only for Principal Investigators (PI) who are NOT affiliated with Hunter College.

2. Initial HRPP/IRB approval letter MUST be attached.

3. Attach a copy of the current Human Subjects Research (HSR) training and education certificate for ALL research personnel.
**CUNY PIs – This must be your current CITI certificate.**

4. ALL recruitment materials, consent forms, and data collection tools MUST be attached.
