The purpose of this form is to collect the information needed for the Alternate Responsible Officer to process your DS-2019, which is required for prospective J-1 students planning to study in the United States for one or two semesters. As part of the U.S. visa application process, you must complete and submit the DS-2019 Request Form along with all required supporting documents. Please carefully read the instructions provided in our email and ensure that all required documents are submitted by the stated deadline. Pay close attention to details such as minimum insurance coverage amounts and acceptable forms of financial proof to ensure your materials meet the requirements.
To receive your DS-2019, you must submit the following documents:
1. Photocopy of your passport 2. Exchange Visitor DS-2019 Request Form 3. Financial documentation demonstrating how you will meet educational and living expenses in the U.S. 4. Proof of health/medical insurance for the duration of your stay in the U.S. 5. Dependents (if applicable)
