This form is to provide us with your new U.S. address so it can be updated in SEVIS. Whenever you change your place of residence, you have within 10 days to report your change of address by filling out an AR-11 USCIS.gov form and mailing it or fill out this form and the DSO will update it in SEVIS on your behalf.
