All students receiving a Committee Letter or Letter Packet must submit the following documents to the Pre-Health Advising Office via this upload form. All file names must include in the name of the application and applicant in the file title. (i.e. File: LastName.FirstName.Application-Type. 2022-2023) 1. A copy of your SUBMITTED health professional school application (AMCAS, AACOMAS, AADSAS, OptomCAS, or VMCAS). The application must include the submission date. 2. A Letter Request form (AMCAS only) with the correct fila name format (i.e. File: LastName.FirstName.AMCAS Letter Request). For all other schools, the evaluation request should be requested from the CAS Evaluation Portal using the hunter.phcl@gmail.com email (review the instructions sent for the contact person information) 3. A copy of your standardized test scores (MCAT, DAT, GRE, and OAT) MUST also be provided to the Pre-Health Office. Please upload your test scores as PDF’s with the name of the test and applicant in the file title and email subject (i.e. File: LastName.FirstName.Test-Score). The uploading of the committee letter is contingent upon you having released your test scores and application data to the Pre-Health Advising Office. Documents must be submitted in PDF format. Failure to upload your documents in the specified format will delay the uploading of your Committee Letter/Letter Packet.
