1. Attend all scheduled tutorial sessions and keep the tutor informed of all coursework.

2. Come to each tutoring session prepared.

3. Notify Ms. Carlixta Hernandez (che0003@hunter.cuny.edu) and the tutor if you are unable to attend a session.

4. Notify Ms. Carlixta Hernandez if you no longer wish to receive tutoring.

5. Complete a tutor evaluation form at the end of each session.

6. If you do not attend three scheduled appointments, your appointment will be canceled.
