Did the replacing officer take the 'Orientation Quiz'?

Did the replacing officer take the 'Orientation Quiz'? Yes No

If the replacing officer did not take the orientation quiz, please have them go to the clubs registration instruction page and click the link to complete said quiz.
Did the replacing officer contact the CA office regarding the CA workshop/quiz?

Did the replacing officer contact the CA office regarding the CA workshop/quiz? Yes No

If the replacing officer did not contact the CA office, they must do so at hcca214@hunter.cuny.edu
Upload replacing officers SPARC Certificate of Completion.

If an officer resigned, please upload their resignation.
